Alumni Transcript Requests
Background
Former John Stark Regional High School students can easily request the release of their official transcript by downloading the form below. Official transcripts may be needed for college applications, scholarships, athletics (NCAA), summer programs, etc.
Requests for transcripts must be made at least TWO WEEKS prior to your deadline.
Official transcripts are sent directly to the school or agency requested. Please note that any transcript sent directly to the student will be marked Unofficial.
Procedure
- Download the form (linked below).
- Complete the form.
- Return to the JSRHS Counseling Center via mail, fax or email.
